Well Visit and Vaccine Schedule
Lake Worth Pediatrics is committed to delivering the highest quality of preventive and interventional health care to children from birth through early adulthood. One of the most fundamental services we can provide to our patients is vaccinations against life threatening diseases. We strongly agree with the American Academy of Pediatrics (AAP) and Centers for Disease Control (CDC) that childhood vaccines are critical to maintaining healthy children and communities. Based on current medical literature and scientific studies, we firmly believe in the safety and effectiveness of vaccines to prevent serious diseases and to avoid the outbreak of such diseases community-wide. Therefore, we feel that vaccinating children following the recommended schedule is absolutely the right thing to do for all children and young adults. In accordance with these views, our new policy requires that every patient in our practice be given all vaccines as mandated by the AAP, CDC and State of Florida.
Routine visits are scheduled at specific stages of your child’s life. Below please find the schedule, along with the recommended immunizations, tests, and procedures.
Newborn/First Week
• Review hospital stay and screening tests (please bring your newborn’s hospital discharge papers)
• Establish baseline growth parameters
• Physical exam
• Vaccines: 1st dose of Hepatitis B (if not given at hospital)
2 Weeks (if needed)
• Weight check
• Discuss results of newborn screening
• Jaundice follow-up
1 Month
• Monitor growth and development
• Physical exam
• Vaccines: 2nd dose of Hepatitis B
2 Months
• Monitor growth and development
• Physical exam
• Vaccines: 1st dose of DTaP, HiB, Pneumococcal (PCV20), Polio (IPV), Rotavirus (oral)
4 Months
• Monitor growth and development
• Physical exam
• Vaccines: 2nd dose of DTaP, HiB, Pneumococcal (PCV20), Polio (IPV), Rotavirus (oral)
6 Months
• Monitor growth and development
• Physical exam
• Vaccines: 3rd dose of DTaP, HiB, Pneumococcal (PCV20), Rotavirus (oral)
• Optional: Influenza vaccine during flu season (Oct-Feb)
9 Months
• Monitor growth and development
• Physical exam
• Vaccines: 3rd dose of Hepatitis B, Polio (IPV)
• Optional: Influenza vaccine during flu season (Oct-Feb)
12 Months/1 Year
• Monitor growth and development
• Physical exam
• Screening for anemia and lead level
• Vaccines: 1st dose of MMR, Chickenpox (Varicella), Hepatitis A
• Optional: Influenza vaccine during flu season (Oct-Feb)
18 Months
• Monitor growth and development
• Physical exam
• Vaccines: 4th dose of Pneumococcal (PCV20), 2nd dose of Hepatitis A
• Screening for Autism
• Optional: Influenza vaccine during flu season (Oct-Feb)
24 Months/2 Years
• Monitor growth and development
• Screening for Autism
• Physical exam
• Screening for anemia and lead level
• Vaccines: 2nd dose of Hepatitis A (if not previously given)
• Optional: Influenza vaccine during flu season (Oct-Feb)
30 Months
• Monitor growth and development
• Physical exam
• Optional: Influenza vaccine during flu season (Oct-Feb)
3 Years
• Monitor growth and development

• Screening for vision and hearing
• Optional: Influenza vaccine during flu season (Oct-Feb)
4 Years
• Monitor growth and development

5-10 Years

11 Years
• Monitor growth and development
• Screening bloodwork
• School physical
• Vaccines: Tdap
• Optional: Meningococcal ACWY, HPV series (Gardasil), Influenza vaccine during flu season (Oct-Feb)
12-15 Years
• Monitor growth and development
• School Physical
• Optional: Influenza vaccine during flu season (Oct-Feb)
16 Years
• Monitor growth and development
• School Physical
• Vaccines: 2nd dose of Meningococcal ACWY
• Optional: Meningococcal B series, Influenza vaccine during flu season (Oct-Feb)
17-21 Years
• Monitor growth and development
• School Physical
• Optional: Influenza vaccine during flu season (Oct-Feb)
*Beginning January 1st, 2020, existing patients/parents who continue to decline required vaccines despite all our efforts will be asked to find another healthcare provider due to our compassionate concern for all children. Although herd immunity may allow the impression of natural protection, please recognize that by not vaccinating you are putting your child and those around you, especially the most vulnerable, at unnecessary risk for life threatening illness, disability, and even death. Also, please be aware there are many web sites that contain misinformation about vaccines. We encourage you to consult legitimate, science-based sources such as the ones linked below.
