As a new patient, you will be asked to complete the New Patient Packet that includes our registration form, medical records release form, our HIPAA privacy policy as well as our financial and medical consent forms. The New Patient Packet comes in a Spanish version as well. We kindly request forms to be downloaded and completed before the appointment time.

For newborn babies, we encourage the parents to contact the insurance companies as soon as possible after hospital discharge. Please be aware that most health insurance plans allow 30 days to add your newborn on the policy.
If you have coverage through Medicaid, you must contact the Medicaid office to receive and activate child's member identification number. Make your Primary care Physician selection at the same time to avoid any delays in providing the medical care to your newborn.
If your child does not have any medical coverage, you will be required to pay for the services.

Policies
Our patients, newborns through adolescents, are seen by appointment for illness, injury, and well child examinations. You can request same day or weekend appointment. We also offer evening hours Monday through Thursday by request only.
If you must cancel an appointment, please notify us 24 hours in advance. There is no charge for the first missed appointment, however, if there are repeated missed appointments, without prior cancellation, subsequent office visits will not be scheduled.
We respect the time spent in the waiting room and we make every effort to minimize any delay. There are situations some patients are seen on a priority basis based on emergency conditions. We hope you understand any delay in your appointment under such circumstances.
